IMPORTANT INFORMATION:

VITA ( Volunteer Income Tax Assistance) is a free tax preparation service for individuals and families whose household income is $60,000 or less.

IF APPILICABLE, PLEASE BRING WITH YOU:

• Social Security cards for yourself, spouse and dependents
• Proof of identity (US Drivers License, Employer ID)
• All tax forms, W2, 1089, etc.
• Information for other income.
• Identification number for childcare provider and amount paid
• Receipt for college tuituion paid
• Proof of account for direct deposit (voided check)
• School ID, State ID, Military ID, Passport
